Successfully promote and support the mission of Catholic Charities of Northeast Kansas as guided by the Catholic Social Teachings.  Works as part of the Interdisciplinary team and performs selected nursing care in accordance with plan of care, under the direction of the RN Case Manager and Clinical Supervisor. As a full-time Float, the LPN should be flexible in order to meet the changing demands of Hospice.


  • LPN Float provides skilled hospice nursing care to patients as directed by the Registered Nurse and IDT plan of care, in accordance with agency policies.
  • LPN Float promotes and maintains patient quality of life under the guidance of the Registered Nurse who is assigned to the same case.
  • Assists patients in learning appropriate self-care techniques.
  • Helps to achieve and maintain continuity of patient care by assisting in planning and exchanging information with other health professionals and team members.
  • Prepares clinical visit notes and follows client care plans as developed by the registered nurse and according to agency policy.
  • Assists the physician and Registered Nurse in performing specialized procedures for which competency has been established.
  • Keeps abreast of changes/trends affecting hospice nursing care and licensed nurse functions.
  • Provides coverage as needed in areas requesting assistance, including team visits, vigil care and crisis care to meet the needs of Hospice patients/families.
  • Communicates current health status and needs of clients to appropriate health professionals and team member.


  • Valid KS/MO LPN license.

  • Minimum one year hospice experience required.
  • Demonstrated knowledge and skills necessary to provide care and communication.
  • Demonstrated knowledge of hospice philosophy.
  • Able to assess data reflecting the client's status and interpret the appropriate information needed to identify each patient's requirements.
  • Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients.  Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching.  Visual acuity and hearing to perform required nursing skills.
  • Valid driver's license and required auto liability insurance.
