Catholic Community Hospice is looking for a PRN Hospice Chaplain to successfully promote and support the mission of Catholic Charities of Northeast Kansas as guided by the Catholic Social Teachings. The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Catholic Community Hospice, either directly or through coordination of care with other spiritual counselors and will occur during the evening and/or weekends. Essential Job Functions/Responsibilities 

  1. Facilitates the development in the interdisciplinary team/group plan of care to meet identified spiritual needs.
  2.  Provides direct spiritual care to patients and families/caregivers. 
  3. Serves as liaison and support to community chaplains and spiritual counselors. 
  4. Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision. 
  5. Documents direct services and ongoing communication with community chaplains and spiritual counselors. 
  6. Attends patient care conference and interdisciplinary team/group meetings as a member of the interdisciplinary team/group. 
  7. Provides consultation, education, and support to the interdisciplinary team/group on spiritual care. 
  8. Provides for funeral or memorial services for patients as requested. 
  9. Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.  
  10. Accepts and performs other related duties and responsibilities as required. 

Position Qualifications 

  1. Graduate of accredited seminary or school of theology (BD, M.Div. or equivalent theological degree). Experience working with death and dying, individuals/family/caregiver.

  3. Ability to work as member of interdisciplinary team/group. 
  4. Comfort in an interfaith setting. 
  5. Ability to accept different lifestyles, cultures, beliefs, and values. 
  6. Ability to network with community clergy and congregations. 
  7. Knowledge of and commitment to hospice philosophy of care. 
  8. Healthcare setting and Hospice experience preferred.
